Records Preparation

The company’s Claim form for Insurance Benefits has been filled out completely and accurately.

Death certificate/ Certificate of Death (if available) and ID card, Permanent Residency of the deceased.

The autopsy report/ A record of cause of death;

Medical documents, accident documents as mentioned above (depending on the case of death due to illness or accident);

ID card or permanent residency of the Insurance Buyer / Beneficiary;

Legitimate certificate of the representative of the Insurance Buyer / Beneficiary Insurance Buyer / Beneficiary does not appear in the name of the person requesting the Claim for Insurance Benefits, and ID card or Permanent Residency of the legitimate certificate of the representative (as the case may be);

Legal heir(s) certificate of the Insurance Buyer/ Insured person/ Beneficiary or the authorized person by the inheritance of the Insurance Buyer/ Insured person/ Beneficiary if there is no Insurance Buyer/ Insured person /Beneficiary or the Insurance Buyer/ Insured person /Beneficiary has deceased, and ID card, Permanent Residency of the legal heir(s) (as the case may be);

Authorization letter and ID card, Permanent Residency of the authorized person (if authorized)

A written request to become the Insurance Buyer, agreement on the new Insurance Buyer, agreement on the Beneficiary of the Insurance (as the case may be).

The company’s Claim form for Insurance Benefits (Form 02-GQQLBH) has been filled out completely and accurately;

ID Card, Permanent Residency of the Insured person/ the Insurance Buyer;

Authorization letter (Form 04.a- GQQLBH/ Form 04.b- GQQLBH), and ID Card, Permanent Residency of the authorized person (if authorized);

Medical document: Injury Certificate/ disability status of the Insured person issued by the competent authority; Medical summary, Film and X-ray / CT / MRI results, Hospital checkout, Patient transfer form, A detailed list of medical expenses, Hospital bill, Prescription, Health records and relevant medical documents.

Note:
- If the Insurance Buyer/ the Insured person was completely paralyzed, the diagnosis of disability must be made in six (6) months but no more than 12 (twelve) months from the date of the accident or the date on which the condition was diagnosed.
- If the Insurance Buyer/ the Insured person has completely lost arm(s) or foot or eye(s) (as defined in the Product Terms of Service), the diagnosis of disability can be done at any time.

The company’s Claim form for Insurance Benefits (Form 01-GQQLBH) has been filled out completely and accurately

ID Card, Permanent Residency of the Insured person/ the Insurance Buyer;

Authorization letter (Form 04.a- GQQLBH/ Form 04.b- GQQLBH), and ID Card, Permanent Residency of the authorized person (if authorized);

Medical document (particularly the documents which demonstrated the recovery immediately after the accident): Medical summary, Film and X-ray / CT / MRI results, Hospital checkout/ Surgical certificate (if available), Patient transfer form, A detailed list of medical expenses, Hospital bill, Prescription, Health records and relevant medical documents.

Accident document: Confirmation of accident (certified by the police, local authorities about the date and time of the accident); Scene examination Report, Accident Report, Accident diagram, Accident settlement Report and relevant documents.
